Appointment Reminder Messaging Program

Program Name: Sunset Park Community Services Appointment Reminder Messaging Program

The Practice offers an appointment reminder text messaging program for individuals who provide a mobile telephone number and consent to receive appointment-related text messages.

By opting in to the Practice’s appointment reminder messaging program, you may receive text messages concerning your appointments and related scheduling activities.

These messages may include:

  • Appointment reminders;

  • Appointment confirmations;

  • Appointment scheduling information;

  • Appointment rescheduling information;

  • Appointment cancellation notifications; and

  • Other text messages directly related to the administration of your appointments and services with the Practice.

The messaging program is intended for service-related communications and is not a marketing or promotional messaging program.

Consent to Receive Text Messages

By providing your mobile telephone number and affirmatively opting in to receive appointment reminder text messages, you consent to receive text messages from Sunset Park Community Services regarding your appointments.

Your consent to receive appointment reminder text messages is not a condition of purchasing goods or services.

You understand that appointment reminder text messages are sent to the mobile telephone number you provide and that you are responsible for ensuring that the telephone number provided to the Practice is accurate and current.

If you provide a mobile telephone number that belongs to another individual, you represent that you have authorization to provide that number and to receive communications at that number.

Message Frequency

Message frequency will vary based on your appointment schedule and your interactions with the Practice.

You may receive messages when an appointment is scheduled, approaching, confirmed, changed, rescheduled, or canceled.

The number of messages you receive may therefore vary and cannot be guaranteed.

Message and Data Rates

Message and data rates may apply.

The Practice does not charge a separate fee for appointment reminder text messages. However, your mobile carrier or wireless service provider may charge you for text messages, data usage, or other telecommunications services.

You are responsible for any applicable charges imposed by your mobile carrier or service provider.

How to Opt Out

You may opt out of receiving appointment reminder text messages at any time by replying STOP to an appointment reminder text message.

After you send “STOP,” you may receive one final text message confirming that your request to opt out has been processed.

After opting out, you will no longer receive appointment reminder text messages unless you subsequently provide consent to receive them.

You may also contact the Practice directly to request assistance with opting out of appointment reminder text messages.

Opting out of text messages does not cancel or otherwise change any scheduled appointment. If you opt out of appointment reminders, you remain responsible for keeping track of your appointments and contacting the Practice regarding scheduling questions, cancellations, or changes.
